acknowledges and agrees that Esperia Volleyball Academy may collect, use, and store their personal information provided in this waiver agreement for the purposes of administering this agreement, managing their participation in activities at the Facility, and providing information regarding services and updates related to the Facility. Esperia Volleyball Academy will handle the Client's personal information in accordance with applicable data protection laws and regulations, and will not disclose it to third parties without the Client's consent, except as required by law

I declare that the info I’ve provided is accurate & complete. I hereby acknowledge and agrees that providing false or misleading information is illegal and may result in legal consequences.

I hereby acknowledge this release from liability for accidental injury or illness which I may incur as a result of participating in any physical activity. I hereby assume all risks connected therewith and consent to participate in this program. I agree to disclose my physical limitations, disabilities, ailments, or impairments which may affect my ability to participate in this program.
